Building the Bridge to Better Care Collaboration and Coordination

Last week, our team attended the Healthcare Information and Management Systems Society (HIMSS) conference in Las Vegas. With over 40,000 attendees, 1,300 vendors, and 300 education sessions offered, there was no shortage of content and networking. Our team had the opportunity to engage with several healthcare systems to talk about the important role that the long-term post-acute care (LTPAC) market plays when it comes to care coordination, an area that typically hasn’t been a major part of the conversation in the past.

However, while attending HIMSS, we noticed a palpable shift. The level of interest and number of conversations in regards to the role LTPAC can play in the continuum have increased. More healthcare systems are seeking out information on post-acute providers and how they can help drive greater business success.

There’s a lot of change happening across the healthcare industry and, now more than ever, it’s important to empower health systems with the insights needed for a truly coordinated and collaborative approach to care delivery. Last week, I presented on this topic and was able to share how our team is working to achieve this level of visibility across the care continuum.

We are honored to call over 15,000 LTPAC Providers and more than 60% of all the Skilled Nursing Facilities in the US clients and partners. Our vision is to become a representative voice for the LTPAC market and our partners, and to bring our piece of the quality care delivery puzzle to the forefront. The LTPAC market plays a vital role in improving care outcomes and enabling health systems to meet the Fee-for-Outcome model requirements, and with the single largest repository of post-acute data of any long-term care vendor, PointClickCare is well positioned to provide the patient insights required for true coordinated care.

To help bridge this gap, and to allow for an exchange of post-acute patient information in a simplified and secure way, PointClickCare announced two significant partnerships during the conference. The first is with Redox Inc., the leading integration platform for healthcare data exchange, which will enable us to connect with healthcare systems, empowering them with the visibility required for a truly coordinated approach to care.

Our other new partnership is with Salesforce Health Cloud. Through this, health systems will gain access to data about post-acute providers in their caregiver networks, enabling them to select partners that will deliver a higher quality of care for better patient outcomes.

For LTPAC providers, sharing data with acute care facilities allows them to demonstrate their ability to deliver high-quality care, which can help attract more referrals. Additionally, having a more direct connection will make patient transfers much smoother by delivering important information before the patient even arrives at their facility. Having the right insights, such as the types of medications that have been prescribed or any therapy treatments that have been requested, will help LTPAC providers better prepare for incoming patients and ensure the highest quality of care at all times.

Overall, last week’s HIMSS conference was a success. It provided our team the ability to educate healthcare systems on the important role the LTPAC market plays in care delivery, and allowed us to connect with various industry leaders and develop the necessary relationships needed to be able to move the needle when it comes to improving care coordination.
