Bridging the Gap between Hospitals, ACOs, and You

Do you know what hospitals are looking for? How about ACOs? We know these topics are top of mind lately which is why we hosted a Leadership Luncheon to discuss bridging the gap between hospitals, ACOs, and you. To position this topic for maximum exposure, we held an informative session on data-driven conversations and a panel discussion on preferred providers.

With narrowing networks becoming a reality, having data that speaks to your capabilities as a healthcare partner is critical for senior care providers. Preparing for and enabling those conversations is a key strategic activity. Our session “Having Data-Driven Conversations with your Healthcare Partners,” discussed what providers and vendors are doing to help enable care coordination conversations across the care continuum and how data-driven approaches to episodic care management practices and value-based payment models are being envisioned. The key takeaways of this session were: understanding what other providers are doing right now and where the gaps are, strategies you can take to build relationships, and how to leverage technology/tools to help you prepare the conversation with networks.

We then hosted a panel discussion on ‘Preferred Provider Status: What are Hospitals and ACOs Really Looking For?’ which was centered around what qualifies a provider to achieve a spot in a hospital’s or ACO’s Preferred Provider Network. Attendees were able to hear directly from our panel of hospital representatives as they discussed their view on exactly what they look for from facilities. The panel included: Beth Israel Deaconess Care Organization (BIDCO), Atrius Health, South Shore Health, Partners HealthCare, Queen Anne Nursing Home, and BaneCare Management LLC. They also discussed ways that we can improve clinical outcomes and performance through data sharing. This engaging panel discussion helped attendees to understand what kind of data is being used between hospitals, SNFs, and ACOs, how that data is being used, and how they can improve clinical outcomes and performance with data.

Feedback on the event was highly positive and 100% of attendees said they would be interested in attending a similar event in the future.
