British Columbia experiences over 10,000 transfers between the long-term care (LTC) and acute sectors each year. Research indicates that more than 20% of LTC residents are readmitted to acute care within 30 days. This is due to a reliance on manual paper and fax processes, which can result in higher potential for medication errors, difficulty in obtaining accurate diagnoses, and ultimately longer wait times. PointClickCare’s Project AMPLIFI addresses these challenges by streamlining transitions in care through the facilitation of bidirectional clinical data exchange between long-term care facilities and hospital-based information systems.
