What are ADT Notifications?

ADT notifications serve as alerts that are sent when a patient is admitted to a hospital, transferred to another facility, or discharged from the hospital. These alerts notify a patient’s primary care physician and other members of the patient’s care team to ensure proper care coordination and follow-up, and to prevent unnecessary hospital readmissions and other avoidable gaps in the care continuum.

The presentation drew on firsthand experience in planning and implementing connected care and quality initiatives that enhance coordination capabilities, and help care coordinators and doctors manage the health of individuals and populations. It touched on observations about the value of ADT notifications gleaned over the past decade.

It also powered FLHIE’s Event Notification Service, DHIN’s Encounter Notification System, UHIN’s cHIE alerts, CRISP’s Encounter Notification System, and HSX’s Encounter Notification Service.
