Big News From SUMMIT!

Big news! We were so excited to introduce our vision for integrated care coordination this morning and share that our customers will have access to the data they need through the Carequality framework as part of their existing base subscription. Carequality is a trusted system that enables meaningful data exchange among health data sharing networks.  

Our goal is to help our customers ensure more seamless continuity of care for their patients through quickly reconciled medications and diagnoses, with the goal of reducing readmissions and driving care plans forward. 

This move means that we are facilitating data exchange between our customers and the hospital and care partners within their networks. Enabling this connection will help our customers optimize patient care, and given the size of our footprint (60% of the Skilled Nursing market), we are doing it at a larger scale than anyone else in the healthcare industry.  

PointClickCare is the only LTPAC provider to make this connection available.
