Reducing Network Leakage with Post-Acute Interoperability: Not Just a Health Plan Problem


Network leakage occurs when a hospital discharges a patient to a post-acute setting and the post-acute partner does not refer the patient back to the original hospital. The anchor hospital will often receive a readmission penalty and miss the opportunity to re-treat the readmitting diagnosis.

  This scenario not only creates missed revenue opportunities, but the patient is also done a disservice as critical information is often lost through this misalignment in referral patterns.
