Focus on Your Residents’ Goals and Wishes
You need to know what your residents’ objectives are in order to provide them with the best care possible and help them to achieve their discharge goals. What does discharge look like for them? Are they going home? Somewhere else? This information is key because knowing where they will go next will help to determine the best course of action.

Standardized Care
Standardization is the epitome of the work smarter, not harder concept. By implementing standards-based care, you are able to create a framework that is used with every resident, by every nurse. This creates a consistent care process, which can easily predict outcomes and ultimately will lead the resident to success.  

Maintain Close Relationships with Hospital Partners
Facilities that maintain a closer partnership with hospitals have a reduced risk of their residents experiencing a rehospitalization. Why is that? Plainly stated, because creating a clear line of communication and providing all the necessary information to both parties ensures that everyone involved has everything they need to help the resident get better.
