How to Optimize Your Resident’s Care Stay

Admissions is just one piece of your resident’s experience. Once you’ve mastered the process outlined in  our previous blog post, “6 Steps to Optimizing Your Admission Process”, you might be wondering what comes next.

In our next eBook “How to Drive Consistent Care Delivery” we focus on the next phase in your resident’s experience — ensuring their safety and wellness throughout their entire stay.

So, how do you do that? Well, it can be a bit challenging and there are a few ways to approach the care stay phase. Our new eBook details multiple ways to ensure your residents’ care stay is a positive experience for everyone involved. Here’s some of what you’ll learn in the eBook, “How to Drive Consistent Care Delivery”.

Key Points for Optimizing Care Stay:

Managing Bills and Reimbursements
You might be thinking, ‘how does that help the resident?’ In a Skilled Nursing Facility (SNF), reimbursement is tied to the MDS. With PDPM coming soon, understanding the connection to payer and reimbursement is integral to getting paid. Knowing that payments are taken care of allows your resident to concentrate on getting better and not their bills.

Focus on Your Residents’ Goals and Wishes
You need to know what your residents’ objectives are in order to provide them with the best care possible and help them to achieve their discharge goals. What does discharge look like for them? Are they going home? Somewhere else? This information is key because knowing where they will go next will help to determine the best course of action.

Standardized Care
Standardization is the epitome of the work smarter, not harder concept. By implementing standards-based care, you are able to create a framework that is used with every resident, by every nurse. This creates a consistent care process, which can easily predict outcomes and ultimately will lead the resident to success.

Maintain Close Relationships with Hospital Partners
Facilities that maintain a closer partnership with hospitals have a reduced risk of their residents experiencing a rehospitalization. Why is that? Plainly stated, because creating a clear line of communication and providing all the necessary information to both parties ensures that everyone involved has everything they need to help the resident get better.
