These resources have been compiled to support the implementation of the RoP for Skilled Nursing Facilities originally effective November 28, 2017.
Reflecting the changes effective August 8th, 2024, the Facility Assessment Template is updated. The Facility Assessment Template is a downloadable Word Document formatted for ease of use and identifies the reports and additional information you will need to collect in order to complete the form. Download it on its own or with the accompanying instructions. We hope that these resources help you in getting ready for your completion of the facility assessment.
